چکیده

A CT scan demonstrated a subarachnoid hemorrhage. A followup four-vessel cerebral angiogram demonstrated bilateral anomalous ACAs, with the origin at the level of the ophthalmic artery proximal to the posterior communicating and anterior choroidal arteries. The proximal portion of each ectopic proximal segment was horizontal , suggesting that both ACAs coursed below the optic nerves (Figs . 1 A and 2A). In addition, there was a broad-based saccular aneurysm off the proximal segment of the right ACA, pointing anteriorly and inferiorly below the optic nerve (Fig . 18). The anterior communicating artery (ACoA) was slightly high and posterior in location , and there was a saccular ACoA aneurysm pointing upward (Fig . 28).

An Unruptured Anterior Communicating Artery Aneurysm with Bilateral Infraoptic Anterior Cerebral Arteries. Case Report and Review of the Literature

Variations of the anterior cerebral artery-anterior communicating artery complex are commonly identified in aneurysm surgery. An infraoptic course of the anterior cerebral artery is exceedingly rare. Robison first described this anomaly from an anatomic dissection in 1959. A unilateral anomalous infraoptic anterior cerebral artery is more common than anomalies of bilateral infraoptic anterior c...

Infraoptic Course of the Anterior Cerebral Artery: Case Report

(ACA)-anterior communicating artery (AComA) complex are commonly identified at imaging studies, an infraoptic ACA is extremely rare (1-4). In such cases, an anomalous vessel originates from the internal carotid artery (ICA) at the level of the ophthalmic artery, and then pursues an infraoptic and prechiasmatic path to supply the ACA. We describe a case of in which this anomaly coexisted with a ...

Dissecting Aneurysm at the Proximal Segment of the Anterior Cerebral Artery Associated with Infraoptic Course Anterior Cerebral Artery

A 48-year-old man presented a subarachnoid hemorrhage caused by a rupture of a dissecting aneurysm at the proximal segment (A1 segment) of the right anterior cerebral artery (ACA). He also had an anomalous artery named infraoptic course ACA and an agenesis of the contralateral ACA A1 segment. Balloon occlusion test at the bifurcation of the right internal carotid artery demonstrated that the di...

A Ruptured Aneurysm at the Infraoptic Azygous Anterior Cerebral Artery with the Contralateral Internal Carotid Artery Agenesis Treated by Y-stent Assisted Coil Embolization

Infraoptic anterior cerebral artery (ACA) is an extremely rare congenital anomaly. This anomalous artery usually arises from the intradural internal carotid artery (ICA) near the level of the ophthalmic artery (OA) or rarely from the extradural ICA. This anomaly frequently harbors a cerebral aneurysm, and may involve other coexisting vascular anomalies. In the case of this anomaly, surgical tre...

Coil embolization of an aneurysm associated with an infraoptic anterior cerebral artery in a child.

We report an unusual case of a ruptured aneurysm from an infraoptic anterior cerebral artery in an 11-year-old child. To our knowledge, this is the second such case treated with Guglielmi detachable coils in the pediatric population that has been described in the literature, excluding that of a vein of Galen aneurysm that resulted in remarkable recovery. We conclude that the endovascular techni...
